Create your free account on the Jane App with the email you used to schedule your intake appointment! Your New Patient Paperwork will be waiting there for you. Email any test results (MRI, X-Ray, bone density scans, blood work, etc.) to us at Office@Adjust-ology.com and we will upload them to your file. If you have any difficulty getting any of your records, let us know and our office staff can obtain a signed medical release form from you and request the information directly from your health-care practitioners.
